Grant Request Guidelines

Please submit the following information to the CBWCF by April 1 to the address below.

1) Description of organization – mission statement, goals and objectives.
2) Current documentation regarding 501 (c) 3 status.
3) Community impact of organization – program services description.
4) Educational component – how does the organization tie into the mission of the CBWCF?
5) Dollar amount of funds requested.
6) Specific use of grant funds – description of issue or need, anticipated outcome.
7) Number of volunteers available to assist with CBWCF events. (Optional at CBWCF‘s request).
8) Copy of organization’s latest tax return.
9) List of current board members.
